Purpose of the Tool

The purpose of this tool is to assist your educational entity in preparing for an
on-site Office for Civil Rights Review. You can use this tool to review your school's policies and practices and prepare for an on-site review.

Directions for Use:

Step 1:  Download the Report Form (doc). You will need this in order to record your responses.

Step 2:  Answer questions listed in the Standards section. Record your answers on the Report Form.

Step 3:  Obtain additional information to assist you in answering questions from the Resources and Legislative Resources sections.

Step 4:  Save and print your answers on the Report Form.
